Electronic Admission Forms

Please find below six electronic forms to support your child’s admission to Walton High School.  The deadline for completion is Monday, 22^nd June 2026.

Please ensure that you have all the relevant information BEFORE you start to complete the forms as you will not be able to save and return to individual forms once you start to complete each one.

You will need:

• Contact details (email, address, mobile number) for all contacts who have parental responsibility
• Contact details for third contact (if you need a third contact)
